FAQs about the Insomnia Treatment Program

  • Most of the science and art of treating insomnia is in understanding the problem with all of its nuances. Every patient has unique biology, circumstances, history, and preferences. To make sure you benefit as much as possible from the program, we do a thorough assessment (no cutting corners). This is part of what makes our treatment program different from sleep apps and coaching programs.

  • Yes, you will be working one-on-one with either Dr. Miller or Dr. Wu. Both of them are Board-certified behavioral sleep medicine specialists. Your provider will personally see your case through from assessment to final maintenance planning. She will personally monitor your data and provide feedback throughout the program, and meet with you via video telehealth if these sessions are part of your treatment plan.

You will not be chatting with an AI bot or unlicensed coach at any point during treatment.

  • Cognitive behavioral treatment for insomnia (CBT-I) was traditionally conducted in hour-long sessions face-to-face. More recently, there have been some evidence-based self-guided apps. Our program creates a best-of-both-worlds approach that is time-efficient and clinically powerful:

    • Allows time flexibility. You can engage with the treatment materials at any time of day instead of scheduling follow-up appointments with your provider.

    • Allows closer and more timely monitoring. Your provider will see your data as soon as you enter it, and provide feedback without having to wait until the next appointment.

    • Allows nuanced personalization. There is a human expert behind every treatment recommendation from your provider, who knows your case well. You will collaborate with her to assess progress and make modifications in real time.

  • In our clinical experience, most people will be satisfied with their improvement in 8 weeks. But if you need more support, or want to address a related issue (e.g., anxiety, nightmares) after the 8-week insomnia program, we will be happy to work with you.

Additional face-to-face video sessions will cost $140/ 30-min session.

Ongoing data monitoring and treatment recommendations (without video) will cost $100/week.

  • We understand that sometimes life throws a curve ball, or you simply don’t feel like the program is a good fit. Please see the following for how we can handle these situations so you get the most out of the program without feeling stuck.

    Refund and Withdrawal Policy

The intake assessment fee of $390 is non-refundable once the intake session has been completed, as it reflects clinical assessment, case formulation, and treatment planning work performed by your clinician.

Early withdrawal (within 7 days of intake): You may withdraw from the program within 7 calendar days of your intake session and receive a refund of the treatment portion of your fee, less a $200 administrative fee.

After 7 days: Treatment fees are non-refundable. However, you may pause your treatment at any time and resume any time at no additional cost. To pause, notify your clinician or our admin team in writing. (If the pause is more than 6 months long, your clinician may need to re-do the assessment or conduct a video session check-in to make sure the treatment plan is still the right one for you).

Clinical discontinuation: If your clinician determines that the COAST program is not clinically appropriate for your needs, or you’ve been advised by another healthcare provider to discontinue for a clinical reason, you will receive a refund for the unused portion of your treatment fee.

Non-engagement: Refunds are not available for non-engagement with the program. If you are struggling to engage, please contact your clinician — pausing is always available as an alternative to discontinuation.

  • Dr. Miller and Dr. Wu each host a 1-hour office hour each month (total of 2 office hours per month). Patients will have access to this “ask me anything” style informal group session for 1 year from starting their Insomnia Treatment Program (for those participating in the relevant tiers). This allows people to get live answers without having to book a full appointment session during their treatment maintenance phase.

  • You don’t have to get off of sleep medications to start the Insomnia Treatment Program. In fact, people improve best when they hold their sleep medications steady during the acute CBT-I treatment phase.

But if getting off sleep medications is a goal, we can absolutely build this into the treatment plan. Usually, we start by making behavioral/physiological changes, then start tapering off of sleep medications when your sleep improves and you feel confident about moving forward. Most people do very well with this goal once they have established their foundational tool box for addressing insomnia.

*Important: Do not make any changes to how you take your sleep medications (including over-the-counter medications) without supervision from your prescribing healthcare provider. With some sleep medications, stopping too quickly can be dangerous or can increase sleep problems.

  • The 8-week program costs between $1190 and $2410, depending on the frequency of live video sessions and how long you want access to “office hours” after the treatment program is done.

Please see our Services page for more details.

FAQs about behavioral sleep medicine (BSM) and our treatment approaches

  • Behavioral sleep medicine (BSM) refers to a collection of evidence-based approaches for assessing and treating sleep-wake disorders. Assessment involves a sleep specialist using a range of methods to understand your sleep problems in the context of your physical/mental health, physical/social environment, and other factors. Treatment involves you collaborating with your provider on non-medication approaches to improving sleep, such as changing your nighttime and daytime behaviors, customizing your daily schedule, reframing the way you think about sleep, and refining your living environment and coping skills.

For example, cognitive-behavioral therapy for insomnia (CBT-I) is a BSM treatment, and it is highly effective for adults with insomnia. Most people with sleep problems can benefit from BSM approaches, especially if they are interested in decreasing their use of sleep medications.

  • Cognitive-behavioral therapy for insomnia (CBT-I) is one treatment under the BSM umbrella and the gold standard treatment for adult insomnia. American Academy of Sleep Medicine recommends it more strongly than it recommends all sleep medications (including over-the-counter ones) because it has consistently demonstrated high efficacy and low risk in over 200 high-quality clinical trials. In practice, it involves:

-Tracking your sleep and learning to understand factors that contribute to your insomnia

-Learning the science of sleep to let go of misconceptions that have kept you stuck in unhelpful patterns

-Changing daytime and nighttime behaviors to increase sleepiness and decrease arousal (e.g., racing mind)

-Practicing skills and gaining knowledge for sustaining sleep health in the long term, including during stressful times

-Decreasing use of sleep medications, if this is your goal

  • Yes, we help people with a range of sleep and sleep-related problems:

    • Managing sleep apnea

    • Nightmares

    • Coping with hypersomnia or narcolepsy

    • Anxiety and depression

    • Stress

Please see our Services page to learn more.

  • We do not prescribe medications.

However, most patients come to us already on sleep medications, and we are happy to work with each person’s specific goals. Sometimes this means getting off of medications altogether, and sometimes it means streamlining medications with collaboration with your prescribing healthcare provider.

One of the major strengths of behavioral sleep medicine (BSM) is that it uses behavioral and psychological strategies to change your sleep physiology. This makes it safer, more effective, and longer-lasting than medication treatments for insomnia.
