Insomnia Treatment Program
Our 8-week insomnia treatment program combines our years of clinical experience with an evidence-based tech platform to help you improve your sleep and start living life to the fullest. It is rooted in cognitive behavioral therapy for insomnia (CBT-I), which is the only treatment for insomnia that receives the American College of Physicians’ and American Academy of Sleep Medicine’s “Strong” recommendation, ahead of any sleep medication. We personalize treatment for each patient by adding, modifying, or re-sequencing treatment tools to meet specific needs. To make treatment accessible and to triage effectively, we offer three levels of treatment that vary in live face-to-face time spent with a sleep specialist:

Most of the science and art of treating insomnia is in understanding the problem with all of its nuances. Every patient has unique biology, circumstances, history, and preferences. To make sure you benefit as much as possible from the program, we do a thorough assessment (no cutting corners). This is part of what makes our treatment program different from sleep apps and coaching programs.

Yes, you will be working one-on-one with either Dr. Miller or Dr. Wu. Both of them are Board-certified behavioral sleep medicine specialists. Your provider will personally see your case through from assessment to final maintenance planning. She will personally monitor your data and provide feedback throughout the program, and meet with you via video telehealth if these sessions are part of your treatment plan.
You will not be chatting with an AI bot or unlicensed coach at any point during treatment.

Cognitive behavioral treatment for insomnia (CBT-I) was traditionally conducted in hour-long sessions face-to-face. More recently, there have been some evidence-based self-guided apps. Our program creates a best-of-both-worlds approach that is time-efficient and clinically powerful:
Allows time flexibility. You can engage with the treatment materials at any time of day instead of scheduling follow-up appointments with your provider.
Allows closer and more timely monitoring. Your provider will see your data as soon as you enter it, and provide feedback without having to wait until the next appointment.
Allows nuanced personalization. There is a human expert behind every treatment recommendation from your provider, who knows your case well. You will collaborate with her to assess progress and make modifications in real time.

In our clinical experience, most people will be satisfied with their improvement in 8 weeks. But if you need more support, or want to address a related issue (e.g., anxiety, nightmares) after the 8-week insomnia program, we will be happy to work with you.
Additional face-to-face video sessions will cost $140/ 30-min session.
Ongoing data monitoring and treatment recommendations (without video) will cost $100/week. -
We understand that sometimes life throws a curve ball, or you simply don’t feel like the program is a good fit. Please see the following for how we can handle these situations so you get the most out of the program without feeling stuck.
Refund and Withdrawal Policy
The intake assessment fee of $390 is non-refundable once the intake session has been completed, as it reflects clinical assessment, case formulation, and treatment planning work performed by your clinician.
Early withdrawal (within 7 days of intake): You may withdraw from the program within 7 calendar days of your intake session and receive a refund of the treatment portion of your fee, less a $200 administrative fee.
After 7 days: Treatment fees are non-refundable. However, you may pause your treatment at any time and resume any time at no additional cost. To pause, notify your clinician or our admin team in writing. (If the pause is more than 6 months long, your clinician may need to re-do the assessment or conduct a video session check-in to make sure the treatment plan is still the right one for you).
Clinical discontinuation: If your clinician determines that the COAST program is not clinically appropriate for your needs, or you’ve been advised by another healthcare provider to discontinue for a clinical reason, you will receive a refund for the unused portion of your treatment fee.
Non-engagement: Refunds are not available for non-engagement with the program. If you are struggling to engage, please contact your clinician — pausing is always available as an alternative to discontinuation.

Dr. Miller and Dr. Wu each host a 1-hour office hour each month (total of 2 office hours per month). Patients will have access to this “ask me anything” style informal group session for 1 year from starting their Insomnia Treatment Program (for those participating in the relevant tiers). This allows people to get live answers without having to book a full appointment session during their treatment maintenance phase.

You don’t have to get off of sleep medications to start the Insomnia Treatment Program. In fact, people improve best when they hold their sleep medications steady during the acute CBT-I treatment phase.
But if getting off sleep medications is a goal, we can absolutely build this into the treatment plan. Usually, we start by making behavioral/physiological changes, then start tapering off of sleep medications when your sleep improves and you feel confident about moving forward. Most people do very well with this goal once they have established their foundational tool box for addressing insomnia.

Managing Obstructive Sleep Apnea
Obstructive sleep apnea (OSA) can feel like a heavy, wet blanket. It keeps your sleep chronically fragmented and increases risk for heart and brain health problems. Although we don’t diagnose OSA or prescribe CPAP (and related device/surgical interventions), we help people with the important work of managing this chronic disease. This includes getting used to CPAP, managing co-occurring insomnia, and improving daytime functioning.

Shift Work and Circadian Rhythm Disorders
When your biological clock is thrown off rhythm by jetlag, shift work, or trying to live as a hardwired night owl in a morning person’s world (i.e., delayed sleep phase)… it’s impossible to feel good or function well. We use strategic sleep-wake scheduling, light therapy, and medication timing to help your circadian clock align with your work/social obligations, or to create a sustainable sleep-wake system that allows you to get the most out of life.

Nightmares
Nightmaring is a learned behavior for the brain. That means it can be unlearned. Overcoming frequent or distressing nightmares can improve your overall sleep, decrease anxiety/depression, and decrease symptoms of posttraumatic stress disorder (PTSD). Using imagery rehearsal therapy (IRT), the American Academy of Sleep Medicine recommended treatment, we can help you to gain control over your dream experience.
